By clicking on the above link you will be able to see an alphabetical list of drugs, with that drug’s category (e.g, generic, preferred brand) shown to the immediate right, and with the share of that drug’s cost that you will be expected to pay on the far right. So when the list indicates Lyrica is “Preferred Brand Name” drug, you’ll see to the right the indication that your share of the cost for a 30 day supply, filled at a retail pharmacy location, is 25%, with a minimum of $20.

In the event category shown to the right of a drug is labeled “excluded”, it means that drug is not part of the Plan’s covered formulary. In such a case you can contact your physician, your pharmacist, or ProAct, regarding available alternatives to that drug.
