Automatic Premium Reminders / Payments

Have Text Message Reminders of Health Premiums Due Sent to Your Cell Phone – With One Simple Step
You can elect to receive text message reminders whenever your health care premiums are due. To receive these reminders simply log on to our Self-Service portal, click on the ‘Contact Information’ tab and under ‘Communication Preferences’ check the box ‘Receive Text Message Reminders of when Health Premiums are Due’ and click on the ‘Submit’ button.

Once you have signed up, reminders will be scheduled for all of your premiums and any monthly premiums due. Naturally the Fund Office cannot be responsible for messages not transmitted or received (though we will make every reasonable effort to assure that does not happen). Please note that some text messages may be generated even if you have paid, so feel free to ignore them if you are sure that the Fund Office has received your payment. Finally, please be aware that while this is a free service from the Fund Office, standard text message rates from your phone service provider will apply.

DIRECT PAY PROGRAM BETWEEN THE EQUITY-LEAGUE HEALTH TRUST FUND AND THE ACTORS FEDERAL CREDIT UNION

In order to help you avoid losing health coverage because you don’t pay your health premiums by their deadlines, the Equity-League Health Trust Fund and the Actors Federal Credit Union (ActorsFCU) have established a Direct Pay Program that allows you to have your health premiums paid automatically through ActorsFCU. Under this program, your health premiums, as billed to you by Equity-League, will be deducted automatically from your ActorsFCU checking account. Equity-League will coordinate with ActorsFCU and provide them with the billed premium amounts that you see on your billing invoice.

All you have to do to enroll in this optional program is to download the automatic payment authorization form located on the ActorsFCU website, www.actorsfcu.com (the form is currently under Special Offers), and then return it to ActorsFCU.  You can also call them at 1-800-2-ACTORS, or pick up this form by visiting the Credit Union’s New York or Los Angeles offices.

The Premiums you can pay under this program are:

• $100.00 Quarterly Contribution

• COBRA Coverage

• All other Self-Pay Coverage –Self-Pay Program After COBRA, Dependent Coverage (Through Covered Employment), Medicare Supplemental, and Dental Only.

You will then authorize ActorsFCU to pay your premium and the amount will be deducted from your account. If you do not have sufficient funds in the designated account, ActorsFCU will make up the difference, with overdraft protection of up to $750.00 (for a modest fee), through their Courtesy Pay Program. That is, if you don’t have enough in your account to make the required payment(s), ActorsFCU will make up the difference. Please note that this does not apply to those who have declined this service. Deductions from your account will occur approximately the first of the month prior to the start of your calendar quarterly coverage period – January, April, July, and October. Therefore, the payment transactions will take place during the months of March, June, September, and December.
