For the 2023 tax year, Federal Law does not require individuals to provide proof of health insurance coverage via a 1095-B form. Therefore, the Equity-League Health Fund will no longer be automatically mailing plan participants copies of that form.
However, if you had Cigna medical coverage in 2023, the 1095-B form can be downloaded on our website, through your online Self-Service Portal account at portal.equityleague.org. You can currently locate it under the Health Payment Tab, by clicking on the 1095-B tax form link and choosing the one for 2023. If you are not already a registered account user, please register and then follow the steps described above. You can also request that a paper form be sent to you, by contacting the Equity-League Benefit Services Department via email, health@equityleague.org or by phone, 212-869-9380/toll free, 800-344-5220. You can also mail in a written request and send it to:

Mailings of the 1095-B paper form will still automatically be sent to those who at any time were residents of the four states that still require the 1095-B – California, New Jersey, Rhode Island and Washington DC. These mailings occurred prior to January 31, 2024. A copy can also be obtained by accessing it through the self-service portal that was stated above.
If you had HMO medical coverage in 2023 with Kaiser or Emblem Health (HIP) New York, please contact the HMO carrier directly to request a copy of the 2023 1095-B form for your records:
For Emblem Health (HIP):

For Kaiser:
- Go to https://healthy.kaiserpermanente.org
- To contact Kaiser directly for this information, you can do so by calling 1-844-477-0450, and a representative will assist you.
Kaiser D.C, Northern and Southern California also already mailed out the required 1095-B statements at the end of January 2024 for anyone who had HMO coverage in 2023 and resided in Washington, DC and California.
