Federal Law no longer requires individuals to provide proof of health insurance coverage via a 1095-B form. Therefore, the Equity-League Health Fund no longer mails plan participants copies of that form. If you had coverage through Cigna or Kaiser in 2025, you can get a copy of your 2025 1095-B form for your records.
