Accessing Your 1095-B Form

If you had Cigna medical coverage in 2025, a copy of your 1095-B form can be downloaded through your online Self-Service Portal account.

If you have already registered, you will need to log in and then you can locate it under the Health Payment Tab, by clicking on the 1095-B tax form link and choosing the one for 2025.

If you are not already a registered account user, please register and then follow the steps described above. You can also request that a copy be sent to you by mail or email, by contacting the Equity-League Benefit Services Department:

The Form 1095-B will still be mailed to those who reside in states that still require it (including California, Massachusetts, New Jersey, Rhode Island and Washington, DC). This mailing will occur before January 31, 2026. If you want a copy of your 1095-B sooner than that, you can obtain one through the self-service portal.

If you had HMO medical coverage in 2025 with Kaiser, please contact Kaiser directly to request a copy of the 2025 1095-B form for your records:
